Benchmarking Breakout: 28-day readmission, what does good look like?

Event Description

In this one-hour wānanga to explore 28-day readmission indicator information.  

We’ll ask the usual questions such as: 

• How does our district’s 28-day readmission rate compare with others across the motu? 
• What factors might be contributing to these results? 
• What opportunities do the data highlight for quality improvement?
• What actions are your services currently taking to reduce 28-day readmissions?

But also

• What else do we need to consider when designing improvement in our 28-day readmission rate?
• If we imagine what good looks like for people in the future, what is an acceptable 28-day readmission rate?
• Should there be a national benchmark rate (target) for 28-day readmission?

Benchmarking Breakouts are facilitated in a wānanga style, encouraging active participation, shared learning and open discussion. These sessions provide a welcoming and safe space to ask questions, share experiences and connect with colleagues from across Aotearoa.
