The Child Health Integrated Response Pathways (CHIRP) model aims to prevent children from bouncing between services and falling between gaps. The model recognises the unique contribution each team makes to the care of children and whānau. The model also recognises spaces where services can work together to address complex needs, as well as where services could reconfigure to address needs that more than one team could manage on their own.
CHIRP goals ensure that all Tamariki and their whānau, who are experiencing behavioural, neuro-developmental or mental health concerns experience timely, responsive and integrated care that matters to them.
This session will be facilitated by the interdisciplinary team at Hutt Valley – Stacey Jamieson (Team Leader, ICAFS), Dr. Jess Allen (Community and Developmental Paediatrician), Grace Garnham (Manager, Child Development Service) and Dr. David Chinn (Consultant Child and Adolescent Psychiatrist, ICAFS).
